If the form reads like it was written only for lawyers, it isn't doing its job for the patient.

Who can legally give consent on a medical consent form?

It depends on the patient. A competent adult signs for themselves; for a minor, a parent or legal guardian signs; and for an adult who can't decide for themselves, a healthcare proxy or someone with medical power of attorney does. Some states also let emancipated or older minors consent to certain care, such as reproductive or mental-health treatment. This is general information rather than legal advice, and the rules vary by state, so confirm who's authorized before someone signs.

Can medical consent be withdrawn after the form is signed?

Yes. Consent to medical care can generally be revoked at any time — even after signing and even partway through treatment — because a signature isn't a permanent commitment. The cleaner approach is to put the withdrawal in writing and tell the provider so it's documented. This is general guidance, though, and a procedure that's already underway can be more nuanced.

How long does a medical consent form stay valid?
